Use [ Rectangle Tool (U) ] to create a new rectangle.
Right click on its layer and click on [ Resterize Layer ].
Use Warp to apply settings below.
Result:
Making Layer Style
Since style of this layer is similar to previous layer, you can use previous layer style by pasting its layer.
Your result should be like image below
Now set opacity of this layer to 50%.
Result:
Press Ctrl + T to change dimension of layer.
Result:
Hold Alt key and drag layer to make a copy of rectangle.
change its opacity to 100%.
Press CTRL+T and change its dimension.
Your result should be like this:
