Ok, Create a new document.

Click on the [ Rectangle Tool (U) ] to create a rectangle.





Right click on its layer and click on [ Resterize Layer ].



Click on the [ Polygonal Lasso Tool (L) ]

Make a selection as shown below.



Go to Select > Inverse or press Shift + Ctrl + I and press delete key on your keyboard.

Make its Style
Double click the layer or go to Layers->Blending options and apply settings as shown.
